What the FDA labels say about taking them together
Sirolimus and trandolapril
Trandolapril's FDA label of October 31, 2025 names sirolimus in its drug interactions section; sirolimus's label does not name trandolapril itself but warns about its class (ACE inhibitors). The trandolapril label: “Mammalian Target of Rapamycin (mTOR) Inhibitors Patients taking concomitant mTOR inhibitor therapy may be at increased risk for angioedema.”

What does the sirolimus label say about trandolapril?
Sirolimus's label does not name trandolapril itself; it warns about its class (ACE inhibitors):
Sirolimus label, warnings and precautionsThe concomitant use of sirolimus with other drugs known to cause angioedema, such as angiotensin-converting enzyme (ACE) inhibitors, may increase the risk of developing angioedema.
Sirolimus label, warnings and precautionsElevated sirolimus levels (with/without concomitant ACE inhibitors) may also potentiate angioedema [ see Drug Interactions ( 7.2 ) ].

What does the trandolapril label say about sirolimus?
Trandolapril label, drug interactionsMammalian Target of Rapamycin (mTOR) Inhibitors Patients taking concomitant mTOR inhibitor (e.g., temsirolimus, sirolimus, everolimus) therapy may be at increased risk for angioedema ( see WARNINGS – Head and Neck Angioedema ).
Trandolapril label, warningsWhere there is involvement of the tongue, glottis, or larynx, likely to cause airway obstruction, emergency therapy, including but not limited to subcutaneous epinephrine solution 1:1,000 (0.3 to 0.5 mL) should be promptly administered. (See PRECAUTIONS - Information for Patients and ADVERSE REACTIONS .) Patients receiving coadministration of an ACE inhibitor with an mTOR (mammalian target of rapamycin) inhibitor (e.g., temsirolimus, sirolimus, everolimus) or a neprilysin inhibitor (e.g., sacubitril) may be at increased risk for angioedema.
