What the FDA labels say about taking them together
Amiloride and olmesartan
Olmesartan's FDA label of September 23, 2025 names amiloride in its drug interactions section; amiloride's label does not name olmesartan itself but warns about its class (ARBs (angiotensin II receptor blockers)). The olmesartan label: “Concomitant use of olmesartan with other agents that block the renin-angiotensin system, potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium or other drugs that may increase …”

What does the amiloride label say about olmesartan?
Amiloride's label does not name olmesartan itself; it warns about its class (ARBs (angiotensin II receptor blockers)):
Amiloride label, warningsThe risk of hyperkalemia may be increased when potassium-conserving agents, including amiloride HCl, are administered concomitantly with an angiotensin-converting enzyme inhibitor, an angiotensin II receptor antagonist, cyclosporine or tacrolimus. (See PRECAUTIONS, Drug Interactions .) Warning signs or symptoms of hyperkalemia include paresthesias, muscular weakness, fatigue, flaccid paralysis of the extremities, bradycardia, shock, and ECG abnormalities.

What does the olmesartan label say about amiloride?
Olmesartan label, drug interactionsConcomitant use of olmesartan with other agents that block the renin-angiotensin system, potassium-sparing diuretics (e.g., spironolactone, triamterene, amiloride), potassium supplements, salt substitutes containing potassium or other drugs that may increase potassium levels (e.g., heparin) may lead to increases in serum potassium.
